Housing Type:	Permanent Supportive
Brief Description:	Provides a permanent residence in a building located on State Street in New Haven for individuals who have experienced repeated or extended periods of homelessness and who have mental illness.
Services Offered:	Residence, case management, client groups, individual counseling, mental health referrals.
Eligibility:	Chronically homeless adults (18 or older) with a mental illness.
Application Process:	Call admissions and complete phone screening.
Geographic Area:	Any
Occupancy:	22 units, some of which are exclusively for clients with HIV/AIDS.
Length of Stay:	Permanent
Fees for Services:	30% of income.
Wheelchair Access:	All units.
